Every year students in grades 6-12, in Westfield and Montello School Districts, take a survey on their health behaviors. In the past this survey has been called CHOICES. This year HCHY will be utilizing the survey that is given through the Department of Public Instruction (DPI), Youth Risk Behavior Survey or YRBS.
The questions on the new survey are very similar to the questions from the CHOICES Survey. This survey is completely voluntary and does not count against anything in school. The survey is also completely confidential as no names are placed on any survey so they can’t be identified.
